Health Outreach
Since its inception, the THK Health Outreach project has been developing materials that effectively reach and educate teachers, students, and parents. You can download many of the flyers below and adapt them to your needs. The student incentives can only be ordered by those participating in a THK Health Outreach initiative. To start a project at your school, please contact us.
Teacher Materials
- Teacher True or False: This true or false questionnaire provides teachers with information on the Medi-Cal, Healthy Families, and other low or no cost health insurance programs. This form can be used for parents and other community groups as well.
- Teacher Letter: This is a letter to teachers that comes from the district superintendent and/or the local CTA president explaining the importance of the project.
- Envelopes: Surveys for the parents are placed in these envelopes. These also include a space for the date on which the surveys must be returned.
Parent Materials
- Parent Survey: This survey is given to student to take home to parents. It educates the parents on the health insurance available to them and asks if they are interested in learning more about the no or low cost health insurance options available to them.
Student Materials
- Stickers: These stickers are given to students in K-3rd classes that participate in the project. They are county specific.
- Calculators: These calculators are given to 4th-6th grades who return the surveys to their teachers. They are county specific.
